SOUTHWEST ALLIANCE FOR YOUTH SPORTS ASSOCIATION PARENTAL PERMISSION, WAIVER OF LIABILITY, CODE OF CONDUCT. SOUTHWEST ALLIANCE FOR YOUTH SPORTS ASSOCIATION IS UNDERSTOOD TO INCLUDE ADMINISTRATORS AND BOARD OF DIRECTORS.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND SHALL SERVE AS A
CONTRACT. I FURTHER AGREE THAT MY SIGNATURE SERVES AS ACKNOWLEDGEMENT THAT I HAVE RECEIVED, READ, AND AGREE WITH THE TERMS IN THIS DOCUMENT, AND THAT I SIGN OF MY OWN FREE WILL.
I give permission for my child, a minor, to participate in the SOUTHWEST ALLIANCE FOR YOUTH SPORTS ASSOCIATION, Sports program. I am responsible for the child listed on the team roster for whom I am signing.
Parental Permission for Emergency Treatment: In the event of illness or accident in my presence or any other designated guardian’s presence, I shall be responsible for obtaining care for my child. If I am not present and not able to be reached by phone to provide authorization, I give permission to the coach in charge to obtain any necessary emergency medical treatment (calling 911) and for the receiving facility to secure any and all necessary emergency medical care for my child at my own expense.
Release of Liability: Although the safety of all participants during sporting activities is the priority, participation in sports activities may result in illness, and injuries up to and including death. I expressly assume any risk of illness, injury, and/or death arising from any cause, and agree to waive the right to pursue any claim against the SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association or any of its representatives.
The Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. The name of the child/minor named on the official roster form is the same child/minor who I am signing on behalf of on this document.
Photography/Video Release: I hereby give permission to SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association to photograph and/or videotape my child. The sole purpose of
these photos and/or videos is for private use, publication, advertisement, and exhibition of services by SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association. Photographs and videos remain the property of SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association.
Parent Code of Conduct: I hereby pledge to live up to my responsibilities as a parent participating in this sports program by doing the following:
- I will be a role model of good sportsmanship and character. I will help my child meet his/her
responsibilities to the coach and the team. - I will refrain from the use of alcohol, drugs, or tobacco at all sports events.
- I will be a good sport by cooperating with my coaches, teammates, opponents and officials.
- I will discuss the significance of this Code of Conduct with my family members.
I understand that the penalties for not adhering to this Code of Conduct may range from a verbal warning to expulsion from the activity.
I hereby release, hold covenant not to sue, discharge, and hold harmless the SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions,
or negligence of the SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association, its employees, agents, and representatives before, during, or after attending any SOUTHWEST ALLIANCE FOR YOUTH SPORTS Association game, program, or activity.
